Support Management
Stepping Stones
This school readiness initiative takes a holistic approach towards preparing children for school. Focusing on families with children prenatal through five, our Support Management team will help connect families with the resources and services they need to have children ready to learn when they start school. Serving as the family’s advocate and liaison, Support Managers ensure that families receive the quality service and care that they need. Barriers that normally discourage families are eliminated through consistent follow up and language appropriate representatives. Families living in the attendance area of Captain Jason Dahl, Los Arboles, and Seven Trees Elementary Schools will benefit from our service.
Stepping Stones Services:
- Support Managers work with families to create and coordinate a Family Success Plan
- Families maintain one point of contact for all their family needs.
- Support Managers navigate the complex processes within agencies to ensure families don’t become discouraged and follow through with services
- Language barrier are eliminated through our bilingual staff
- Eliminate duplication of services through strong collaboration with community-based organizations
- Partnership with Catholic Charities and Lenders for Community Development improves economic stability
- Partnership with The Health Trust ensures families in this community receive necessary dental care
- Partnership with Choices for Children and SMART START San Jose provide informative, interactive activities to enhance the quality of child care and a connection to child care licensing opportunities.
